Activity waiver & release
I understand that there may be some inherent risk of injury associated with both mine and my child’s (or children’s) participation in gymnastics and sports-related physical activities (“Activities”) provided by The Little Gym® Europe (operated through The Lifeskills Co SA) and its franchisee The Little Gym® Wimbledon(together “The Little Gym® Europe”). I hereby waive any and all claims of liability and agree to release and hold harmless the Little Gym® Europe in the event that such an injury occurs to myself or my child (or children) as a result of taking part in The Little Gym® Europe’s Activities, unless and to the extent that such injury or death is as a result of the negligence of The Little Gym® Europe or its employees, agents or representatives.
Medical attention waiver
I hereby give my permission to The Little Gym® Wandsworth to procure medical attention for myself or my child (or children) in the event of injury, accident, and/or illness to myself or my child (or children) while participating in The Little Gym® Europe’s Activities which, in the opinion of a paramedic or medically trained practitioner, requires urgent medical attention and a parent, legal guardian or any given emergency contact are not available.
Infectious diseases waiver & release
I voluntarily enter the premises of The Little Gym® Wandsworth with my child (or children) and family members, and acknowledge that there may be inherent risk of exposure to, and contracting, infectious diseases, including but not limited to, influenza & COVID-19 associated with this and my child’s(or children’s) participation in The Little Gym®Europeactivities. I have been encouraged by The Little Gym® Europe to practice good hygiene and appropriate measures including social distancing to reduce the likelihood of contracting or spreading infectious diseases. I have read, understand and agree to adhere to all of the rules, guidelines and procedures ofThe Little Gym® Europe COVID-19 protocol, which have been formulated to provide reasonable measures to avoid the transmission of infectious diseases. I absolve The Little Gym® Europe and The Little Gym® Wandsworth from any liability with regard to the transmission of infectious diseases.
